Job Summary: The LPN provides skilled nursing services to patients in their homes during their end of life in accordance with the physicians’ orders. For this role, you need to be a nurse that is knowledgeable in caring for the terminally ill with the goal of helping patients live their final days with dignity and comfort. Implement an individualized plan of care under the supervision of an RN. Prepare documentation on the nursing services that are provided each visit. Educate patients and caregivers on coping techniques, end of life care, and provide comfort and support. Qualifications, Knowledge, Skills, and Abilities: Current Missouri Licensed Practical Nurse license in good standing. Hospice experience preferred Minimum of one year experience as an LPN within the last three years. Must be flexible with schedule for patients being added for emergency visits. Excellent organizational, computer, and time management skills. Must be a team player with excellent communication skills.
