The Case Manager will provide caseload support for the Lakewood Police-Based Co-Responder team, including case management and/or care coordination, documentation, collateral contacts and outreach to adult and juvenile consumers with legal involvement. Essential Competencies/Duties: Engage consumers and community supports in the strengths-based and trauma-informed treatment progress. Identify risk factors, including lethality. Work constructively with consumers to reach agreed upon outcomes, and coordinate care with internal and external providers and community partners. Effectively respond to consumer needs and problems, initiate, and maintain positive interactions, timely responses to phone calls, email and all other requests. Participate in oversight of data outcomes/reporting for the program. De-escalation and crisis intervention over the phone, risk assessment for suicidal and homicidal ideation, psychoeducation for families and consumers. Connect consumers and their families to applicable services and resources. Assist with care coordination, case management, and reporting to the Lakewood Police Department. Assist the Lakewood PD Co-responders with care coordination and case management for whole health needs of consumers. Participate in Evidenced Based Practices and other agency initiatives (MI, CBT, Zero Suicide, and Trauma Informed Care). Participate in regularly scheduled supervision, and utilize supervision by coming prepared, reporting high risk cases, and utilizing a problem-solving approach. Knowledgeable about Jefferson Center and community resources, while continuing to build an up-to-date network of available community resources. Other Duties Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment. Maintain effective interpersonal relations with consumers, peers, supervisors, management, community partners and the general public. Attend mandatory in-services, trainings, staffings and consultations as scheduled. Complete 95% of all paperwork by the due date (progress notes, CCARS, and Service Plans). Document in accordance with regulatory requirements and clinical guidelines. Demonstrate knowledge and skills to develop a therapeutic alliance with consumers, and demonstrate cultural competence in working with consumers from diverse backgrounds. Participate in CAS group supervision/consultation per OBH regulations (CAS, 2 hours/month; LAC, 1 hour each month.) As a case manager with the Lakewood Police Department, you will have the unique opportunity to outreach people from every demographic, level of acuity, and walk of life. In this position, you will be able to utilize your case management skills by working with a variety of populations.A typical day involves receiving and triaging referrals from LPD, outreaching community members via phone from said referrals to assess for needs and maintaining intensive case management clients.Note: Employees are held accountable for all duties of this job. Education, Knowledge, Skills & Experience Required: Conferred Bachelor's degree (from an accredited University/College) in psychology, social work, or related field required. Bi-lingual preferred but not required Computer skills (Excel, Word, email, etc.) required Background check required, substance use testing Salary Grade 50 - Bachelor's Degree (Case Manager) $39,100 to $49,800* (18.80/hr - $23.94/hr) Additional Salary Information*: All salaries at Jefferson Center are calculated based on years of total relevant experience.* The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.* Additional pay provided for evening, overnight, and weekend shifts.* *This position qualifies for a limited time grant funded bonus of $1000 paid on the one month anniversary! * Additional Bonus Information*: Based on full time (40 hours/week). Prorate if less than 40 hours/week* Must work a minimum of 20 hours/week to be eligible for bonus* Current Jefferson Center employees not eligible for sign on bonus* Application deadline: 5/21/2024.
