Jefferson Center for Mental Health is seeking a self-motivated, independent skills coach and case manager who can work effectively on a multi-disciplinary team. Someone who enjoys and values the importance of community-based work. Strong boundaries, creative thinking, excellent time management, high-level organization, a positive outlook, and strengths-based approach are necessary for success.This position will provide behavior coaching and case management services to clients ages 4-21 and to families engaged in a high level of therapeutic care, via the Trauma Systems Therapy (TST) Program. The coach/case manager will support the TST treatment process and meet client needs that fall outside of the scope of direct clinical mental health care. They will collaborate closely with the TST therapist and client to meet agreed upon goals and objectives. They will consistently coordinate care with internal and external service providers. The Intensive Family Skills Coach & Case Manager, as part of the Trauma Systems Therapy (TST) Team, will be responsible for the following: Direct Services: Understand how trauma impacts the lives of the people being served, so that every interaction is consistent with the recovery process and reduces the possibility of re-traumatization. Demonstrate ability to work with families with diverse backgrounds including awareness of cultural differences. Support therapeutic work by collaborating with TST therapist and assisting the family in treatment goals. This may include practicing life skills and coping skills, supporting follow through with behavior plans, and supporting parents/guardians. Outreach and effectively plan for sessions to engage client and maximize client benefit from services. Provide regular needs assessments and link clients to relevant services. Collaboratively create case management goals and objectives with client and family. Work to increase and maintain client self-sufficiency by improving client's adaptive level of functioning and use of community supports and natural supports. Attend and effectively participate in meetings with client and family to advocate for client needs. Assess and identify risk factors, including lethality. Report risk factors and concerns to therapist. Support engagement in safety plans created with therapist. Maintain appropriate clinical and work-life boundaries within a family systems treatment approach. Indirect Service & Care Coordination: Coordinate care with internal and external service providers. Exhibit strong communication and collaboration with a multidisciplinary team to address barriers to treatment and advocate for client needs. This may include court hearings, social services staffings, school meetings, or other. Must have strong understanding of professional boundaries in these settings. In collaboration with therapist, identify need to link consumer with medication evaluation when necessary. In collaboration with therapist, refer client to appropriate community providers after TST episode of care is complete. Supervision & Development: Attend team meetings, supervisions, and mandatory in-services as scheduled. Participate in planned individual and weekly group supervision by coming prepared with an agenda. Report high risk/problem cases and utilize a problem-solving approach. Participate in staff development activities to enhance professional growth and follow individualized training plan. Volunteer to work on Center committees and/or task forces. Administrative & Compliance: Maintain appropriate treatment documentation. Comply with all corporate compliance and HIPAA standards, including submitting paperwork on time and meeting Jefferson Center's productivity standards. Complete data tracking required by grant funding sources. Professionalism: Maintain appropriate professional standards of behavior. Timely response to client needs and problems (phone calls, pages, email, and other requests). Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment. Maintain positive and effective interpersonal relationships with clients, peers, subordinates, upper management, visitors, and the public. Work cooperatively with other community agencies, as appropriate, and in agreement with supervisor. NOTE: Employees are held accountable for all duties of this job. Education, Credentials, and other requirements: Bachelor's degree in psychology, social work, or related field required. Two or more years' related experience and/or training required. Reliable transportation required. Work with consumers will regularly take place in the community or in the consumer's home. Non-direct duties may be conducted across a hybrid work setting. Motor Vehicle Insurance Coverage requires applicant must be 25 years or older. Must have an acceptable Motor Vehicle Driving record as determine by our insurance carrier. An MVR will be pulled along with a background check. Must have own personal reliable transportation. Must be able to provide evening sessions at least three evenings per week as needed (last session time offered at 7pm). EXPERIENCE/SKILLS: Ability to connect with families and evaluate needs. Ability to work collaboratively and creatively with a treatment team. Experience providing direct services to clients preferred. Knowledge of local resources preferred. Salary Grade 50 - Bachelor's Degree (Case Manager) $39,100 to $49,800.* Additional Salary Information*: All salaries at Jefferson Center are calculated based on years of total relevant experience.* The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.* *This position qualifies for a limited time grant funded bonus of $1000 paid on the one month anniversary! * Additional Bonus Information*: Based on full time (40 hours/week). Prorate if less than 40 hours/week* Must work a minimum of 20 hours/week to be eligible for bonus* Current Jefferson Center employees not eligible for sign on bonus* Application deadline: 5/1/2024.
