The Primary Care Nurse will function within an interdisciplinary collaborative team and deliver primary care nursing, episodic care, health promotion, chronic disease management and education to clients living in the Inkster/Seven Oaks community and beyond. The position will support primary care, walk in connected care and any primary care site as required. QUALIFICATIONS: Bachelor of Nursing Degree Current active practicing registration with CRNM 2 years recent experience in primary care and/or acute care setting: community experience an asset Excellent nursing skills, including comprehensive backssment and intervention, triage, clinical reasoning, and ability to practice to the full scope Experience and knowledge working with diverse populations including infant, youth, adult and geriatric populations Excellent clinical, communication, counselling, group facilitation and organizational skills Knowledge of health promotion, chronic disease management, case management, adult education and community development Ability to work independently and within an interdisciplinary team environment Must be willing to work weekdays, weekends, evenings and holidays RN-AP (Authorized Prescriber) designation an asset Current CPR certification Child abuse, adult abuse and criminal records check required Valid driver’s license and access to a vehicle Proficient use of MS Office and EMR (Accuro) an asset Forward your application in confidence to: Ashley Henderson Nor West Co-op Community Health Access Nor West, 785 Keewatin Street Winnipeg, MB R2X 3B9 Application Deadline: August 25, 2023 We thank everyone who expresses interest but only those selected for an interview will be contacted.
