Our Case Management Program embodies this belief by supporting individuals experiencing mental illness or addictions to live as independently as possible, while working collaboratively with a Transitional Case Manager to build their support systems. As a Transitional Case Manager, you’ll also focus on assisting clients with connecting to their community and teaching life skills to help them to achieve their recovery goals. The successful candidate must have a Police Vulnerable Sector Check (PVSC) that was completed within the last six months. For candidates who require a request letter in order to apply for a police check, request letters will be provided for candidates that move forward from the interview stage of the recruitment process. WHAT WE'RE OFFERING: What you'll be making - $22.33 to $25.71 per hour (Practicing Professional Grid) The above are initial rates; with grid increases as per policy Internal staff members will be placed on the appropriate grid based on their current rate of pay, up to Level 8 of the grid (see Intranet Policy - bit. ly/Compensation Policy Pg16 ) When you'll work - 70 hours over two-week period, Monday to Friday 8:30am – 4:30pm. These hours may vary on occasion. What your benefits will be - After a 90-day waiting period, will be eligible for extended health care benefits. Your pension plan –Immediate enrollment in the Healthcare of Ontario Pension Plan (HOOPP) PSLRTA Notice - This posting characterizes this position as currently non-union.
