About PACE Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a new non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment. We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care. Position Summary We are looking for a Manager of Process Improvement with energy, expertise, and empathy to work across a complex set of programs, relationships, challenges, and innovations which contribute to service transformation for frail, older adults at Gary & Mary West PACE (Program for All Inclusive Care of the Elderly) (GMW PACE). The successful candidate will have specific expertise in quality and process improvement approaches, methods, and tools to support our collaborative work. Ideally, you will have completed formal quality improvement training such the Institute for Healthcare Improvement's Model for Improvement or hold American Society of Quality or International Association for Six Sigma certification or equivalent. You will be able to think laterally, build strong trusted relationships quickly, and work as a productive team member. You will bring an appetite for analytics, evidence, and learning in everything you do and have an ability to understand new subjects rapidly. The Manager of Process Improvement will work closely with West Health Institute (WHI) researchers, and leaders, and GMW PACE providers, staff, and leaders to implement change management strategies, and execute on process improvement programs to drive clinical and operational efficiency and high-value care delivery at GMW PACE ( https://westpace.net ) The person in this role will lead day-to-day improvement efforts by partnering with WHI and GMW PACE staff to facilitate groundbreaking change and generalizable research outcomes that have the opportunity to be shared more widely with national PACE programs. You will be expected to: Operate in an environment of significant complexity and ambiguity, taking time to understand the context of the operating environment and demonstrating good judgement (including seeking counsel from others when needed); and offer support and guidance for others seeking advice. Exhibit a curiosity about the work going on across the organization, and share with others, internally and externally, impactful work that GMW PACE and WHI support to enable improvement and innovation and resolve complex problems. Key responsibilities include: Lead quality and process improvement teams, change management strategy development, and implementation efforts including establishing and following well-defined transparent project plans to ensure successful and on-time execution. Able to conduct root cause analyses of adverse medical outcomes and high utilization across different disciplines to break down information silos. Proactively identify, track, manage, and escalate action items, risks, issues, decisions, and dependencies. Engage in clear and on-going communication with GMW PACE Executive Director and Medical Director. Represent their goals, perspectives, and needs when/if they are unable to attend meetings (with their approval). Monitor and measure effectiveness of change initiatives and identify areas of improvement, bringing in lessons learned to apply across change management initiatives and process improvement teams. Lead and coach staff and leadership responsible for driving and/or participating in change management efforts. Provide subject matter expertise to teams to ensure change management plans are integrated into project plans, workflows, and protocols. Essential Functions: Work in a dynamic, cross-functional team that drives WHI's strategic plan, support research, and organize external partnerships. Build quality relationships with internal and external stakeholders and exhibit the values of West Health. Lead multiple efforts and teams in quality and process improvement activities to achieve scalable and reliable results. Develop and deliver presentations and training for internal/external consumption. Contribute to the development of assets such as blogs, toolkits, guides, and conference presentations. Ability to seek diverse input from multiple stakeholders to drive innovative solutions. Ability to incorporate feedback and ensure decisions are implemented in a timely matter to support execution. Forward thinking with the ability to recommend, influence, and implement organizational change and continuous innovation. Role model respect and inclusion, creating a culture that fosters innovation. Enable strategies for effecting change, controlling change, and helping people to adapt to change. Proactively identify project issues before they arise and develop contingency plans. Communicate project-related issues to senior leadership and manage issues to resolution. Proven ability to communicate clearly and present key information objectively to all levels of the organization. Function well under pressure in a changing healthcare environment. Education and Certification: Bachelor's degree in health care administration, nursing, public health, or related human services. Master's degree highly preferred. Skills & Experience: Minimum 3+ years of experience using quality improvement framework and methodology. Minimum 3+ years of change management experience leading complex program initiatives across multiple functional stakeholders and teams within a healthcare setting/clinical practice. Strong record of work in process improvement and cultural change management. Demonstrated ability to positively influence outcomes, key project decisions, and strategic problem-solving. Experience in stakeholder relationship Influential in orchestrating key stakeholders and project teams to turn vision into reality. Excellent organizational skills and the ability to manage multiple workstreams concurrently. Desire to make a difference in the lives of older adults. 2+ years of ambulatory, homecare, or hospital experience (ambulatory experience preferred). Experience in geriatrics preferred. Experience providing clinical and rehabilitation care to geriatric populations preferred. Clinical research experience preferred. Knowledge of behavioral healthcare delivery, emergency department utilization management, and chronic disease management preferred. Ability to work independently in a fast-moving environment as part of a multi-disciplinary teams. Strong writer, presenter, and communicator. Has own transportation and the ability to work in multiple locations (primarily San Marcos and La Jolla). Enthusiastic with a "can-do" attitude. Fluent Spanish speaker (a plus).
