VALUES- Teamwork, Excellence, Compassion, Knowledge At Ina Grafton Gage Home, we are passionate about ‘Making Every Moment Matter. ” Responsibilities of the Documentation Nurse at Ina Grafton Gage Home Is responsible for the overall day-to-day delivery of health care operations, ensuring that the health care services comply with the Fixing long Term Care Act (FLTCA) and other relevant legislation. Essential Duties are the technical skills and knowledge that an employee requires in order to perform their role. Below is detailed information on required Essential Duties for your position. Responsibilities and Duties: • Development and modification of the plans of care in response to residents’ individualized health care needs and change in health care status Completes health care backssments (RAI-MDS and other backssments), develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team. Liaises with other health care personnel, agencies and families Maintains good relationships with Medical/Support/Home staff, residents, families and general public Supervises all activities on the unit, takes charge as required • Assists with the MDS Schedules, documentation and monitoring of all informatics on residents backssments • May be required to work on the unit as required • Participates in.
