Job Description: To work as an autonomous specialist practitioner, managing a clinical caseload safely and effectively. Carry out effective specialist holistic backssments and provide specialist care for patients with Lymphoedema, including life limiting conditions. Use shared decision making to empower and support patients and carers to develop person centered short term and long-term self-management plans. Develop and maintain the specialist skills, knowledge and competencies required to backss, treat and evaluate a patient with lymphoedema, including safe use of specialist equipment. Provide mentorship, clinical support and guidance to other Healthcare professionals, junior staff and students, to promote positive patient experience and reduce duplication To follow appropriate local and national policies and guidelines when planning and delivering care. Where appropriate prescribe for patients using independent or Nurse prescribing, or request changes in medication when required from patients GP. To work effectively as part of a small team, providing leadership, support and guidance to other team members and administrative staff. Lead by example and provide a role model for the team, and deputise for Service Lead within own sphere of competence Contribute to setting standards of care, auditing clinical outcomes and service provision. Contribute and support the continuing updating of the service, promoting high quality evidence-based care. Communicate effectively with patients and other stakeholders verbally, in writing and electronically using e-mail. This includes writing clinical records in line with the Sirona Documentation Policy.
