Job summary Seeking a dynamic and talented licensed Social Worker to join our team of compassionate and dedicated clinical professionals. Come and be part our Community Treatment Team. As a team we aim to keep frail patients safe at home and avoid going to Hospital. Main duties of the job Participates as an integral professional member of a multidisciplinary treatment team. Performs social work interventions as directed by the Multi-disciplinary team (MDT). Obtains and backsses social history information and reports this to the MDT. Performs discharge planning/case management from ED Acts as liaison for patients with various community agencies and resources. Attends Multi-disciplinary team meeting to discuss patients social care needs Contribute to the delivery of an effective service as part of a multi-disciplinary team, improving outcomes for adults and carers. Attending the social care needs of patients utilising specialist knowledge for appropriate interventions, needs and risk backssments, care planning and safeguarding investigations. Carry out high quality backssments, using person-centred and multi-agency interventions to bring about positive change through developing effective relationships and to secure the best outcomes for service users and carers.
