Work Location(s): Manatee, DeSoto and Sarasota counties As part of this role, you will: Promote the mission, vision and values of the organization Provide outpatient diabetes self-management training and education to type 1, type 2 and gestational diabetic patients Plan, develop and implement individual and group diabetes education for adults and their families and caregivers Instruct patients in the Standards of Care for diabetes Provide education rotated to diabetes and the disease process; medication, diet and nutritional monitoring; foot care prevention and the techniques of self-monitoring and injection What you need to bring to this role: Bachelor's Degree in social science area; or Registered dietitian or Registered Nurse License with the State of Florida. (A minimum of four years of experience may service to replace the education requirement). CDCES certification preferred Knowledge of nutrition standards and practices Experience in training and providing patient education Excellent communication skills including public speaking Must be able to work flexible hours, which may include evenings and occasional weekends Must have current healthcare provider CPR course completion card Must have or obtain Certified Diabetes Care and Education Specialist (CDCES) within 1 year of hire Proficient in the use of Microsoft Office products, EPM and EHR software Must have reliable transportation to travel throughout Manatee, Desoto, and Sarasota counties Bilingual preferred but not required **MCR Health is a drug free workplace.
