A Behavioral Health Support Specialist (BH Support Specialist) works in the primary care clinic as an integral team member and functions as a member of the Behavioral Health Integrated Services team to provide integrated support to patients with multiple medical and/or co-occurring mental health and/or substance abuse disorders. Advocates for and supports the patient, engages with community agencies/health care providers and others on his or her behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/or hospitalizations. Provides clinical support to the team by providing updates on patient access to care and resources to maintain focus on outcomes and best practices. . ESSENTIAL DUTIES AND RESPONSIBILITIES Assists Behavioral Health Consultants with complex patient cases to ensure access to care. Assists Behavioral Health Consultants with crisis situations, i.e., checking on bed availability, providing current resources to ensure patient safety. Ensures case management codes are properly input into the patients' electronic health record. Follows up with patients and referring agencies to determine access/barriers to care. Assists Behavioral Health Consultants with updating resource materials and information. Remains up to date on resources within the community. Builds partnerships with referral sources and community agencies. Performs other duties as assigned. QUALIFICATIONS Minimum Qualifications: Bachelor's Degree: Social Work, Psychology, or another related field; or related experience.
