We are working closely alongside a Local Authority in Bromley to assist with the appointment of a Social Worker Mental Health , on a 3-month contract , likely to be extended at clients discretion. Please apply with your CV for immediate consideration. Responsibilities: In partnership with adults with identified care and support needs and their carers, ensure that support provided enhances their quality of life, and independence. To work in partnership with all internal and external professionals and organisations involved in backssment and service provision through effective communication and consultation networks ensuring a coordinated response to service users and carers. Provide professional advice and expertise to less experienced staff, including students and colleagues and to work with the complexity casework can produce. Establish, monitor and review care plans agreed with adults, carers and service providers underpinned by mental health and social care legislation, to identify measurable outcomes for adults and carers. Co-ordinate, participate and where appropriate Chair case conferences and planning meetings including Safeguarding meetings. Respond to new and changed legislation in respect of Adult Services and implement as directed. Experience: Experience and working knowledge of mental disorder, Mental Health care and support planning, risk backssment and management. Experience/understanding of staff/student supervision Experience of working effectively with other agencies and professions Experience of reviewing services and reviewing quality and outcomes Experience of working in partnership with service users and carers Experience of working within a statutory setting Experience of dealing with safeguarding issues in relation to adults and children. Experience of contributing to the planning and developing cost efficient services for service users Please note: You should be available to work immediately or at a short notice. .
