Summary: The Care Coordinator performs population health management including risk stratification, enrollment of patients into managed care programs, and management of resources to assist in improving health status. This individual assists in care planning, implementation, data collection, referrals and follow up for patients and populations with chronic conditions in conjunction with the Medical Care Team. Essential Duties : Essential job duties for this position include the following items. Other duties must be performed as assigned or required. Ensures continuity of care for patients across the healthcare continuum, including completion of all documentation in the EMR regarding coordination of services. Risk stratifies patients with standardized tool and documents results in the EMR. Enrolls and manages patient in managed care programs. Collaborates with Finance staff on ensuring accurate billing occurs monthly. Conducts daily or weekly reviews of CCHC chronic care patients and patients enrolled in managed care programs to ensure patients are up to date with age/gender appropriate screenings, chronic disease management follow‐up and scheduling patients when they are due. Supports and empowers patients throughout the healthcare continuum to include holistic counseling, problem solving barriers to care, assisting patients in setting self-management goals around treatment, medication adherence, healthy lifestyle choices and self‐help behaviors. Assesses data reflective of the patient/population's status and requirements relative to age, gender, culture, and disease state. Reports data to monitor patient/population's outcomes and determine action plans. Engages patient and family in developing health care strategies aimed at improving quality of life through education, empowerment, and access to appropriate healthcare services. Connects chronic care patients and patients enrolled in a managed care programs to health education and activities, behavioral health or substance abuse treatment services, and other community resources to ensure that the patient is accessing what he/she needs to improve or maintain health status. Documents work activities, patient correspondence and education with consideration to PCMH, MU, UDS and the Performance Improvement Plan into the EMR. Assists in the collection and reporting of data related to quality improvement initiatives and grant‐related work. Performs quality assurance and utilization management audits of patient charts to assure compliance with various program and grant expectations. Collaborates with the Care Team to develop a standardized plan of care that is evidence‐based for specific disease state management. Collaborates with Outreach and Enrollment to assist with Healthy Behavior Program requirements. Participates in meetings and committees as requested and necessary to perform job requirements. Maintains patient confidentiality and acts as an advocate for patients and families that are underserved. Provides referral coordination and tracking to outside medical agencies. Acts as an advocate for specialty care for patients. Communicates referral status and information to patient in a timely and accurate manner. Tracks referrals and care plan elements in EMR and i2i pertinent to tracking types and other reporting requirements. Documents completion of referrals, chart updates and follow‐up recommendations made by specialists. Work toward continuous quality improvement. Stay current with changing best practices and technologies. Uphold, support, and promote all company policies and procedures. Qualifications: To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills and ability required. Education/Experience: Bachelor's degree in Social Work, Health Education, or related field or equivalent work experience. Experience in Health Coaching and/or motivational interviewing. Certification in Health Coaching preferred. Experience in community health or other public health programs. One year experience in population health management. Experience with an EMR or other health registry systems. Language Skills: Communicate clearly, professionally, and respectfully with all patients and staff. Mathematical Skills: Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Reasoning Ability: Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems. Ability to deal with nonverbal symbolism (formulas, scientific equations, graphs, musical notes, etc.,) in its most difficult phases. Ability to deal with a variety of abstract and concrete variables. Certificates, Licenses, Registrations: If certified, completion of credentialing and privileging requirements. Pre-employment drug testing and background check required. COVID-19 Vaccination of company approved Exemption Required. EOE
