Requirements: Must be licensed as an Occupational Therapist in Arizona Must possess a Valid Drivers License Must possess or be able to obtain Level 1 Arizona Fingerprint Clearance Card Must have reliable transportation Must be willing to travel to members homes or community locations for ongoing treatment Duties: Evaluate individuals for occupational therapy services Develop a Plan of Care based on the members needs and goals allowing them to reach their full potential Review and Update Plan of Care Annually Provide ongoing treatment for individuals as outlined in Plan of Care Document SOAP notes electronically Complete Client Progress Reports electronically Supervise COTA's Effectively communicate with clients, families, administration and COTA's, etc. Excellent written and verbal communication skills Must be a Self-Starter Spanish speaker is a strong plus We look forward to having you join our member centric team of dedicated professionals.
