Trinity is currently hiring a full-time Adult Home and Community Based Service/ CORE Provider, within our Community Network Services Division. To conduct intakes and client centered service plans for referred clients. Service plans will focus on one or more of the following: Psychosocial Rehabilitation; Habilitation; Family Support & Training; Pre-Vocational Services; Education Support Services; Ongoing Supported Employment; Transitional Employment; and Intensive Supportive Employment. What will you be doing as an Adult Home and Community Based Service? Conduct intakes for designated services with clients in approved home and/ or community based setting(s). Coordinate with each client's Health Home Care Manager regarding the frequency, scope and duration components of each client's goals and maintain regular communication with each Care Manager regarding client progress with services and services planning; Complete and submit all required documentation as associated with service delivery and as outlined by each client's Health Home Care Manager, Managed Care Organization (MCO) representatives and/ or agency supervisor(s); Demonstrate excellent time and task management skills, as well as oral and written communication skills. What are your skills and experience? ยท H.S. Diploma or equivalent and 1-3 years of relevant experience working with individuals with substance us disorders and/or serious mental illness or Bachelors degree; Experience working in a home and/ or community-based settings; Able to travel independently within the community; Excellent communication skills; Excellent computer skills, i.e. database management, Microsoft, Excel, etc. Team worker and builder; Flexible work hours to accommodate families and staff ; Must be willing to provide services in participants' homes; Service delivery is designed to be flexible and individualized and is focused on the promotion of skill development and identification of community supports in order to overcome deficits related to function and motivation. Valid NYS driver's license and vehicle insurance coverage Salary - $25.00/hr In addition to a competitive salary, you will enjoy many unique benefits, including 14 paid holidays, access to health insurance at the start of your first full month of employment and you'll immediately start accruing sick days and personal time the moment your employment begins.
