POSITION DESCRIPTION & OVERVIEW Title : Housing Stabilization Case Manager Reports To : Housing Stabilization Support Program Manager Schedule : Mon-Fri, Daytime Flexible, Hybrid Compensation : $20.42-$21.42/Hour Job Classification : Hourly, Non-exempt, 40 Hours/Week Union Representation : This position is represented by AFSCME Local 999 Agate's Housing Programs Department serves households experiencing or at risk of homelessness in Hennepin County. Our programs include Prevention, Diversion, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for households with children and adults without children. By offering programs of various intensity and length, we hope to be able to provide the level of service each household needs to be successful. The goal of our housing programs is to end homelessness by helping households stabilize their housing and achieve self-sufficiency. The Housing Stabilization Case Manager will provide services to support people to live successfully in the community. There are two types of Housing Stabilization Services - Housing Transition Services and Housing Sustaining Services. These services are targeted to people who are homeless or at risk of becoming homeless or living in an institution or other segregated setting or at risk of living in a segregated setting. These individuals will also have or be diagnosed with a disability. The purpose of Housing Stabilization Services is to support an individual's transition into housing, increase long-term stability in the community and avoid future periods of homelessness or institutionalization. Services will be provided in the client's home or in the community. Services provided will strongly emphasize personal goal setting and developing strategies to achieve them, skill building, client advocacy, and connecting clients with community resources. PRIMARY DUTIES AND EXPECTATIONS Meet with individuals 3-4 hours a month in the community or in the individual's home. Ensure that services are connected to the person-centered housing plan developed with each client and delivered in a culturally sensitive and appropriate way following a holistic and person-centered delivery method. Identify and assist with resolving barriers to accessing housing. Provide support in applying for all benefits (county, state, tribal and federal) that will help them afford their housing. Researching and contacting prospective housing options for availability and information (with the Housing Locator) and assisting with applying for housing. Negotiate a lease. Ensure the living arrangement is safe and ready for move-in. Identify resources to cover moving expenses and to access household goods. Supports participants in maintaining housing through prevention and early identification of behaviors that may jeopardize their continued housing. Assist with the housing recertification process both for rental assistance and with negotiating the continuation of a lease. Train and coach on being a good tenant, lease compliance and household management. Support the person to understand and maintain income and benefits necessary to retain housing. Support and coach the building of natural housing supports, services, and resources within the surrounding communities. Provides program and client transportation as needed. Maintain documentation in accordance with Department of Human Services guidelines, in Client Track through weekly progress/encounter notes, monthly progress reports, and file reviews in health records (either paper or electronic). Participate and assist in the tasks associated with client care including evaluation of client safety and appropriate interventions related to their functioning and safety. Acts as an advocate for clients as needed. Provide crisis assistance as needed. Attend trainings and conferences to comply with all continuing education requirements, and further enhance technical and programmatic knowledge. Abide by and complies with the rules of confidentiality, Data Privacy Act, mandatory reporting laws, HIPPA requirements, and organizational policies and procedures. Performs other duties as assigned. Position consists of approximately 60% direct client contact, 30% indirect client-related work and 10% agency- and community-resource engagement. PHYSICAL REQUIREMENTS FOR THIS POSITION Sedentary work - Sitting some of the time with occasional walking. Exerting up to 10 pounds of force occasionally. Community work- Meeting with participants in their community, in spaces where they are comfortable. Must be able to attend home visits and apartment showings with clients, including units that may only be accessible by climbing stairs. REQUIRED QUALIFICATIONS Must have a valid Driver's License Pass a DHS NetStudy 2.0 background check. Successfully completed 30 hours of training during the two years immediately prior to the date of hire or before provision of direct services in all the following areas: recipient rights, person centered planning, mental illness, co-occurring mental illness and substance abuse, local community resources, adult vulnerability, resident confidentiality Must have access to a safe and reliable vehicle. Ability to meet expectations regarding paperwork and documentation through organizational skills, a strong attention to detail, and the ability to work efficiently and accurately, competency and comfort using technology and computers. Ability to handle sensitive situations in a calm, professional manner. Ability to communicate effectively (written and spoken) with co-workers, clients, and outside parties. Strong observation, assessment, and crisis management skills. Ability to handle stress and unstable client behavior. Ability to work independently with minimal direction and within part of a team. Excellent organizational and documentation skills Knowledge of substance abuse and causes and symptoms of mental health issues Experience that gives applicants the required skills, abilities, and perspectives listed above are commonly gained through one or more of the following: Personal experience with homelessness; Experience working, interning, or volunteering with people experiencing homelessness; An Associates, Bachelor's, or Master's degree in a relevant field such as Social Work (with accompanying licensure), Human Services, or Family Studies. This position requires use of your own personal vehicle for Agency business and will be required to maintain liability insurance coverage. In addition to criminal background studies, applicants must have a valid Driver's License and pass a driving record background check. PREFERRED QUALIFICATIONS Two years' experience working with households experiencing homelessness. Licensed Social Worker. Fluency in languages used by our clients (Spanish, Amharic, Oromo, Somali, ASL) AGENCY COVID-19 REQUIREMENTS Agate Housing and Services maintains a COVID-19 preparedness plan that mitigates the transmission and spread of COVID-19. Programs also maintain practices to ensure the safety of staff, clients, residents, and community members. With that, Agate Housing and Services is requiring all new employees, volunteers, interns to show proof of full vaccination upon onboarding.
