Their programmes aim at ensuring quality of life for children and young people in social care. They are made up of a network of professionals with a wealth of experience in the care social industry, this includes independent social work practitioners, care owners and managers, therapists, teachers and senior residential staff. They are looking for a Senior Support Worker to provide Social Work and Care Management services to support young people aged 16+ in semi-independent living. The position involves backssing referrals and designing plans for action and support and assisting the Service Manager with safeguarding investigations. Support the Young People on a day-to-day basis and help them acquire independent living skills. Staff must offer a range of support, including informal counselling, emotional support, advice, assistance with education and training, practical skills training, developing complementary networks and helping the service user address their cultural identity. As the successful candidate, your responsibilities will include : Working with service users where appropriate, in the backssment of individual needs and the provisioning of appropriate social care needs, care management service and local resources available. Acting as the primary point of contact for care arrangements in respect of designated service users with emotional and behavioural difficulties. Working following agreed joint policies and procedures. Identifying the aims and objectives of proposed interventions/placement needs and educational plans Producing care plans and ensuring their effective implementation.
