The Clinical Program Supervisor - REACH Team Lead, works with REACH management team to provide for overall management, supervision and program development of a REACH community based team. Ensures the REACH Team meets productivity, budget, accreditation, and licensure requirements. Develops and maintains working relationships with individuals served, family members and/or care givers, other NRVCS programs, and other agencies working with REACH services. Provides direct services to individuals served.Starting Salary: $70,486-Depending on ExperienceKey Responsibilities: * Ensures the efficient use of fiscal, personnel and other resources for staff supervised as demonstrated by a balanced budget. Assists Program Director with preparation and monitoring of budgets. Monitors REACH Team budget. * Performs at the expected levels of individual and team productivity. Ensures all client records are in compliance with all applicable local, state, and federal requirements including guidelines as they pertain to DBHDS Licensure Regulations, Waiver, State Plan and Clinic Options Medicaid. * Provides clinical and administrative supervision to clinical staff, interns, practicum students and volunteers as needed for licensure supervision purposes. May include chart, paperwork and billing review. Ensures that all personal and staff trainings, certifications and qualifications are current. Ensures all staff have a clear understanding of their job responsibilities and duties. * Assists in recruitment/interviewing for vacant positions. Screens applications, interviews, and recommends individuals to unit director for hire. Provides training/orientation of new staff and interns as needed. * Assists the Director in planning and evaluation of program services, and participates in staff recruitment, training and retention. * Completes formal observations and evaluations of staff's work performance as required by agency policy. Recommends necessary disciplinary action for staff to the Program Director and Human Resources Senior Director. * Manages process for new referrals, ensuring all required documentation is completed accurately and timely manner. Completes and ensures staff complete and update required assessments of needs and strengths to meet the admission and ongoing provision requirements of the service(s) requested and indicated for the individual. Ensures the proper pre-authorization documentation to qualify for and receive payment for all services rendered are submitted within allowable time frames and maintained in the record. * Implements planning and evaluation of program services to ensure that program operates at expected levels of quality. Collaborates with other agencies and NRVCS programs to ensure that all services reflect the individuals' needs and are provided without interruption if possible. Monitors satisfaction with services provided and helps individuals served complete satisfaction surveys as needed. * Ensures all policies and procedures are followed by staff, and recommends and submits needed changes in writing to supervisor. Informs staff of changes in Policy and Procedures as needed. Utilizes authority to improvise and develop appropriate procedures for staff as needed. * Writes effective and measurable Individualized Service Plans (ISP's) for assigned individuals. Works to ensure supports and interventions are provided as stated in the ISP. Responsible for comprehensive review of team's ISP ensuring that plans are written to comply with state, federal, and accreditation standards. Ensures all ISPs are reviewed and modified as necessary to address changing needs of the individuals served. * Assists with crisis resolution and general problem-solving. Ensures services are provided with respect to the dignity of the individuals served as outlined in the State and NRVCS Human Rights Plans. Advocates for services on behalf of individuals served and their families/care givers as needed. Documents and reports suspected violations of Human Rights according to NRVCS policies. * Promotes positive communication and collaboration among team members and other community resources needed to provide quality services. Conducts self in a professional manner at all times. Participates in team meetings unless absence is excused, completes proper written communication and relays other helpful information as necessary by voice mail and/or e-mail. Serves on designated teams, committees and work groups. * Works closely with Emergency Services' staff and other providers to assist with admission process and discharge planning to and from private and public psychiatric hospitals and training centers, including providing support and input to the emergency services providers and judicial system during commitment hearings when appropriate. * Establishes and maintains comprehensive systems for referral and networking of service and resource needs. Works to establish linkages between individuals served and naturally occurring supports in the family and community. Works with team to ensure individuals' health and safety needs are addressed as needed, including medical, dental, psychiatric, psychological, physical therapy, speech therapy, occupational therapy, assistive technology, and environmental modification needs. * Strives to promote continuous quality improvement of services through the timely completion of QA activities (i.e. satisfaction surveys, outcome measurement reports, etc.) and participates in ongoing networking with individuals served, families and/or care givers, other NRVCS programs, and other community agencies and resources as needed. * Ensures all safety, health, and universal precautions are properly addressed and monitored. Completes NRVCS Incident Report forms by the end of the shift for any observed or suspected incidents that involve individuals served or staff welfare and/or safety. Works with Program Director to ensure proper follow-up of all incidents. * Correctly administers medications to individuals supported as prescribed and in accordance with all medication administration regulations. Provides assistance with other medical procedures (i.e.: blood sugar levels, wound care, etc.) as ordered by a physician. Completes a Medication Administration Incident Report Form by the end of shift for any problems that involve medications. Note: Part Time/Relief staff will be trained to administer medication based on program needs. Staff not formally trained and certified by NRVCS may not administer any form of prescribed or over the counter medications. * Ensure adequate staffing is available to support program needs. Provides back-up as needed. Provides on-call and back-up on-call before and after hours as scheduled. Must be accessible by telephone after hours. * Maintains all communications, both written and verbal, with respect and confidentiality and complies with HIPAA and 42-CFR Part 2 standards and regulations. * Ensures all required certifications and/or licensure are kept up to date. Attends/provides other training as required to meet personal professional development and/or program needs. * Completes other related duties as assigned by Program Manager, Program Director and/or Senior Director. Qualifications: * Extensive knowledge and training in best practices for providing community-based crisis services for individuals with co-occurring developmental disabilities and behavioral health diagnoses residing in community settings. * Thorough knowledge of DSM -V etiology, symptoms, treatments and prognoses. Advanced assessment and service plan development skills is required. * Demonstrated ability to lead the work of teams and work effectively with other agencies. * Demonstrated knowledge of agency, state, and federal regulations and standards for REACH, including Medicaid and accreditation standards. * Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment. * Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment; * Must have acceptable drug screen results upon hire. * Ability to manage staff, budget, and program operations effectively and within DMAS guidelines. * Advanced knowledge and understanding of treatment best practices and support modalities for serious mental illnesses and substance use disorders. * Ability to communicate effectively orally and in writing and to prepare and present educational materials. * Ability to establish and maintain effective working relationships with officials, individuals served and their families, co-workers, subordinates and supervisors. Education/Experience Required: * Graduation from an accredited college or university with at least a bachelor's degree in a Virginia DMAS approved human services field, current enrollment in a clinical licensure track Master's program, and at least 5 years of experience working directly with individuals with DD/MH diagnoses is required. * Supervisory experience is preferred. * State of Virginia clinical behavioral health related professional licensure preferred. * If not Licensed, current registration with the Virginia Board of Counseling as a QMHP-A or QMHP-C is required. * Must possess a valid driver's license (Virginia, unless otherwise approved by agency) and an acceptable driving record must be maintained throughout employment. * Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment. * Must have acceptable drug screen results upon hire.
